Register With The Practice
What to do:
- Obtain application form from reception (between 8.00am and 7.00pm Monday to Friday).
- Complete forms clearly and in full If anything is incomplete or unclear we cannot proceed.
- Ensure you have the following information:
- Your NHS number (you can get this from your previous practice)
- Your previous Doctor's name
- The address of your previous practice
- Once original form of identification showing your name and permanent address. This must be dated within the last three months. E.g bank statement or utility bill (NOT passport of driving licence)
- If you have recently moved to a new address and do not have recent proof of identity, please delay your application until this is available.
- Hand in the paperwork in person at reception. Only submit your application when all the above is complete.
- Allow 7-10 working days for the process to be completed.
What happens next:
- All new patients who are over 16 years, will be asked to see the Health Care Assistant for a brief new patient medical. This is to ensure we know about your medical history and gives you to ask any questions.It is necessary because it can take several weeks for your records to transfer to us from your old GP.
- You would normally be unable to see a GP or nurse here until your registration is complete.
- If you require repeat medication soon after registration you must obtain full details from your previous GP and pass it to the receptionist with your written repeat prescription request.
Once registered:
- We have an online service for booking and cancelling rountine appointments with the doctors and repeat ordering of standard items of medication (not changes).
- Please ensure your personal information is always kept up to date and inform us immediately of any changes.
Summary Care Record
If you DO NOT wish to have your basic health information uploaded to the national Summary Care Record (SCR) please complete the relevant form (available from reception) and return it to us with your other paperwork. The basic information includes medications, allergies and adverse reactions only, no other data would be included in the upload. If you do not object to this upload you do not have to complete the form.
NHS Organ Donor Register
If you wish to join the NHS Organ Donor Register you should please go to www.organdonation.nhs.uk and do so online or telephone 0300 123 2323.This is not done at the Practice.
